The financial outlook for Germany's statutory health insurance providers has shown a temporary improvement, as recent figures indicate a surplus for 2025. However, the sector continues to face significant challenges due to escalating expenses, particularly in healthcare services.
According to provisional data released by the Federal Ministry of Health, the 93 public health insurance funds collectively posted a surplus of approximately 3.5 billion euros over the past year. This positive balance comes after a period marked by increased contribution rates for policyholders. The surplus has been primarily allocated to replenish the insurers' financial reserves, which at the close of 2025 stood at around 5.1 billion euros. Despite this increase, reserves remain just below the legally mandated minimum threshold of 0.2 months' worth of expenditures.
In 2025, statutory health insurers recorded revenues of 355.9 billion euros, while expenditures reached 352.4 billion euros. The primary driver behind the substantial increase in costs has been a notable rise in hospital treatment expenses, which grew by 9.6%. Overall, service-related expenditure increased by 7.9%, reflecting a broader trend of rising healthcare costs across the country.
Despite the recent surplus, the financial situation of statutory health insurers remains under strain. Since 2024, expenditures have outpaced revenues, and this trend is expected to persist in the current year. The Ministry has indicated that while the financial situation was stabilized for 2026, and large increases in contribution rates were avoided at the turn of the year, this improvement is only temporary. Projections suggest that from 2027 onwards, the sector could face annual funding gaps amounting to several billion euros.
The government has identified the stabilization of contribution rates as a central policy objective, aiming to reverse the cost trends observed in recent years. An expert commission is scheduled to present proposals by the end of March to address these structural challenges and promote long-term financial stability within the system.
In response to the ongoing financial pressures, numerous health insurers have implemented further adjustments to their additional contribution rates. At the start of 2026, 43 insurers increased their rates, while 47 maintained existing levels. Two health insurers reduced their additional contribution rates at the beginning of the year. These adjustments reflect the continued efforts by insurers to balance rising costs with the need to maintain accessible and sustainable healthcare coverage for the population.
As the sector confronts the dual challenges of increased demand and higher service costs, policymakers and health insurers alike are seeking sustainable solutions to secure the long-term viability of statutory health insurance in Germany.
