The issue of appointment accessibility for patients with statutory health insurance has come under scrutiny as the Bundesrat deliberates on a proposal from Lower Saxony aimed at reforming the current appointment system. This initiative addresses concerns that patients with statutory insurance are often disadvantaged when seeking medical appointments compared to those with private insurance.
The proposal seeks to encourage the federal government to investigate possible disparities in the treatment of insured patients. If inequalities are identified, the initiative suggests implementing measures to ensure fairer access to appointments. These measures could include establishing a minimum quota of appointments reserved for statutory patients or providing financial incentives to physicians who primarily treat these patients.
Lower Saxony's Health Minister has emphasized the necessity for equal access to prompt and high-quality medical care for all citizens, regardless of their income level or insurance status. However, representatives from the Kassenärztliche Bundesvereinigung (KBV), which oversees the interests of physicians in Germany, have dismissed the accusations of private patients obstructing access to appointments. They argue that the majority of patients--approximately 90%--are covered by statutory health insurance, inherently resulting in a greater allocation of appointments for them. Moreover, private patients generally visit doctors less frequently.
Critics of the proposal, including the KBV chair, have characterized it as a superficial solution, asserting that it is not possible to mandate appointments that do not exist or services that lack adequate reimbursement.
In contrast, the Spitzenverband der gesetzlichen Krankenkassen, which represents statutory health insurance providers, has expressed concerns regarding the perceived discrimination faced by their insured patients in appointment scheduling. They advocate for a more equitable system.
Data regarding waiting times for appointments reveals that specialty consultations took an average of 12 days to secure in 2023. General practitioner appointments were the quickest to obtain, averaging four days, while longer waits were noted for specialists such as pediatricians, ophthalmologists, and dermatologists. Endocrinology consultations had the longest average wait time at 26 days.
A survey conducted in 2024 indicated that 31% of statutory patients feel frustrated by the waiting periods for specialist appointments, with a quarter reporting waits exceeding 30 days. Conversely, only 12% of patients reported dissatisfaction with wait times for general practitioners.
KBV representatives have called for a standardized appointment system that accounts for patient no-shows, suggesting that appointments for statutory patients should automatically be compensated. They argue that managing appointments involves more than simply scheduling; it requires coordinating comprehensive care.
Looking ahead, the focus from the Spitzenverband der gesetzlichen Krankenkassen is on developing digital solutions for appointment scheduling that would require physicians to allocate a set number of hours for appointments, irrespective of the patient's insurance type. They argue that true equality in healthcare access cannot be achieved while appointment eligibility continues to be influenced by insurance status. There exists hope that the forthcoming federal government will prioritize this matter and seek effective solutions beyond merely increasing funding for healthcare providers.
