The study drew on data from the National Cancer Database, encompassing over 1.8 million adults aged 18 to 64 who received a cancer diagnosis between 2018 and 2022. Researchers specifically compared the proportions of cancers identified at an early stage before and during the pandemic period, utilizing a difference-in-differences approach to isolate the effect of Medicaid expansion.
Findings demonstrated that states with Medicaid expansion saw notably smaller declines in early-stage cancer detection among several groups. These included adults aged 18 to 44, male patients, individuals with multiple existing health conditions, patients receiving treatment at academic medical centers, and those diagnosed with prostate cancer. The data suggested that Medicaid expansion played a protective role in maintaining access to timely cancer screening and diagnosis during a period when healthcare systems faced significant strain.
The results are particularly relevant as the pandemic led to widespread disruptions in routine healthcare, including delayed screenings and diagnostic services for various cancers. In states without expanded Medicaid coverage, these interruptions contributed to a greater decrease in the detection of cancers at an early, more treatable stage. Conversely, expanded Medicaid eligibility appears to have mitigated some of these negative effects by supporting continued access to health services for vulnerable populations.
The research highlights the importance of health coverage continuity, especially for populations at elevated risk of delayed care during public health emergencies. The authors point out that the findings offer valuable insights for policymakers in the ten states that have yet to expand Medicaid.
