The German government coalition is currently engaged in internal debates regarding its newly introduced savings package for statutory health insurance (GKV). The initiative, led by Federal Health Minister Nina Warken, aims to alleviate financial pressures on health insurance funds by 2027, targeting a relief of 16.3 billion euros, which exceeds the projected deficit of 15.3 billion euros. The core objective is to stabilize the finances of statutory health insurance and avoid further increases in additional contributions for members.
The savings package includes a broad range of cost-saving measures. Proposed actions involve stricter expenditure controls for medical practices, pharmacies, hospitals, and the pharmaceutical sector. Additionally, the plan suggests higher co-payments for medications and restrictions on the free co-insurance of spouses within the statutory health insurance system.
For individuals receiving citizen's benefit (Bürgergeld), who are generally covered by statutory health insurance, the government intends to increase the allocation of tax funds. An initial 250 million euros is designated for 2027, with plans to raise this amount in subsequent years. However, this approach is coupled with a reduction in the regular federal subsidy to health insurance funds, which is set to decrease from the current annual 14.5 billion euros to 12.5 billion euros starting in 2027.
Various stakeholders have expressed concerns about the proposed measures. Medical associations have warned that the planned cuts could negatively impact insured individuals, patients, and healthcare sector employees. They argue that relying on these groups to achieve federal budget consolidation could undermine the principles of fairness and stability within the health system.
Opposition also arises from within the coalition and regional governments. Political leaders at the state level have voiced reservations about the proposed reduction in federal subsidies, warning of potential new funding shortfalls in the healthcare sector. In particular, the suggested changes to free family insurance have been criticized, with calls for further discussions to find more equitable solutions.
The Social Democratic Party (SPD) has indicated a willingness to seek revisions to the package during parliamentary deliberations. One key area of focus is the proposed adjustment to spousal co-insurance, where the latest draft has already reduced the planned co-payment to 2.5 percent, down from the initial suggestion. Nevertheless, SPD representatives are aiming to negotiate the full retention of free co-insurance for spouses during the legislative process.
Additional elements of the savings package include an increase in the pharmacy discount, which is expected to rise from 1.77 euros to 2.07 euros per prescription, with the change taking effect at the start of the new year. This measure is part of a broader effort to curb expenditure growth across the health sector while ensuring the continued provision of essential healthcare services.
As the draft legislation moves forward to the Bundestag, further negotiations and possible amendments are anticipated. The government's goal remains to implement measures that ensure the long-term financial stability of statutory health insurance, balancing fiscal responsibility with the need to maintain high-quality healthcare access for all insured individuals.
