Recent research highlights a concerning correlation between firearm violence in neighborhoods and the decline in dental health among residents. A study conducted by researchers at Rutgers University indicates that higher rates of gun violence are associated with fewer individuals seeking dental care and increased instances of total tooth loss, medically termed edentulism.
The findings, published in the American Journal of Preventive Medicine, utilized data from the Centers for Disease Control and Prevention, focusing on dental care utilization and tooth loss statistics. The researchers assessed data from 20,332 census tracts across the 100 largest cities in the United States between 2014 and 2022, drawing information from both the Behavioral Risk Factor Surveillance System and the American Communities Survey.
According to the study, community-level firearm violence appears to significantly influence dental care usage and overall oral health through several mechanisms. Chronic exposure to such violence can instill fear and stress, adversely affecting individuals' health. For instance, fear stemming from violence may deter residents from pursuing necessary dental care due to concerns for their safety. Additionally, persistent exposure to violence can lead to stress-induced unhealthy behaviors, including smoking, poor sleeping patterns, physical inactivity, and a poor diet--all of which are risk factors for oral health deterioration. Furthermore, the social and economic instability caused by community violence can diminish financial resources and logistical means to access dental care, while also limiting the availability of dental providers in affected areas.
Notably, this study is pioneering in examining the effects of neighborhood gun violence on healthcare utilization. While higher violence rates might ostensibly lead to increased health needs and subsequently, greater healthcare usage, the fear for personal safety often keeps individuals from seeking care, especially dental services which are less frequently utilized compared to general medical services.
Overall, approximately 60% of surveyed residents reported having visited a dentist within the past year, though this percentage varied significantly from as low as 18% to as high as 89%, depending on the specific neighborhood and year. For older adults aged 65 and above, around 15% reported experiencing complete tooth loss annually. In regard to gun violence, neighborhoods averaged one shooting per year, with some areas reporting none and others exceeding 100 shootings in a single year.
The study assessed dental care utilization based on how many adults sought dental services in the past year, which is a crucial metric for preventive health. Additionally, various indicators of community disadvantage were analyzed, such as poverty rates, the prevalence of single-mother households, unemployment rates, reliance on public assistance, and housing cost burdens. A combined disadvantage score was derived from factors like poverty, single-parent families, and unemployment, while other variables were examined independently.
This research underscores the broader implications of community violence, emphasizing that it extends beyond physical harm; it also creates significant barriers to accessing essential healthcare services, such as dental visits, which can have lasting repercussions on oral health.
