Recent research indicates that patients with opioid use disorder (OUD) may benefit from the ongoing use of buprenorphine when experiencing acute pain. This finding emerges from a systematic review published in the Annals of Internal Medicine, which highlights the need for more robust evidence in this area.
A comprehensive analysis led by researchers from Yale School of Medicine evaluated various studies focusing on pain management strategies for individuals facing OUD. The review encompassed 17 trials, 20 controlled observational studies, and 78 uncontrolled observational studies, providing a broad perspective on the subject.
The results suggest that maintaining buprenorphine treatment during acute pain episodes is associated with pain-related outcomes that are either similar or improved compared to instances when buprenorphine is discontinued. The studies primarily drawn upon were conducted in perioperative settings, indicating a potential for buprenorphine to effectively manage pain without compromising the health of patients with OUD.
Additionally, the review identified other pain management interventions that could be beneficial for adults not on OUD medications. These include the use of oral clonidine, intramuscular haloperidol, and midazolam combined with intravenous morphine, as well as intraoperative intravenous lidocaine. However, the findings for these interventions stem from single, well-conducted randomized controlled trials, necessitating further exploration in diverse patient demographics.
It is noteworthy that methadone's efficacy and its influence on OUD outcomes were only evaluated in a limited number of studies, indicating a gap in understanding the broader impact of various pain management strategies on patients with OUD.
The authors of the review emphasize the critical need for additional research into how acute pain management interventions affect outcomes for those with opioid use disorder. This call for further investigation is particularly urgent in light of the escalating crisis surrounding opioid-related overdoses and associated harms.
The findings underscore the importance of tailored pain management approaches for patients with a history of opioid misuse, aiming to balance effective pain relief with the potential risks associated with opioid medications.
