A recent study conducted by researchers from the University of Gothenburg and Uppsala University has revealed a significant presence of titanium microparticles in the oral mucosa surrounding dental implants. This finding emerges from an analysis of tissue samples taken from patients with dental implants, indicating a common occurrence of these particles in the oral environment.
Registry data suggests that approximately 5% of adults in Sweden have dental implants, which raises the possibility of titanium particles being present in the tissue encircling these implants. Although the researchers emphasize that there is no immediate cause for concern, they advocate for further investigation into the long-term effects of these microparticles.
According to Tord Berglundh, a senior professor of periodontology, titanium has been extensively studied and is recognized for its biocompatibility and safety. However, the study highlights the necessity of understanding the behavior of these microparticles over time, specifically whether they remain localized in the tissue or disseminate to other areas of the body.
Previous studies have indicated that titanium particles can accumulate in inflamed tissues around dental implants. The current research, published in the journal Communications Medicine, found titanium microparticles consistently present in tissue samples from all examined implants, including those without any signs of inflammation.
The analysis involved tissue samples from 21 patients, each having multiple adjacent implants. Samples were collected from both healthy implants and those affected by peri-implantitis, an inflammatory condition surrounding the implant. This design allowed each patient to serve as their own control. While the density of titanium particles varied among patients, no significant difference was observed between sites with peri-implantitis and those without within the same individual.
The research team utilized a sophisticated technique known as µ-PIXE (Micro-Proton Induced X-ray Emission) to map the distribution of titanium particles in the tissue samples. This method, which is non-destructive and highly sensitive, provides detailed insights into the elemental composition of the samples, although it requires a particle accelerator for operation. The Tandem Laboratory in Sweden is uniquely equipped to perform µ-PIXE measurements.
To facilitate the study, the researchers enhanced the existing µ-PIXE system, enabling the scanning of larger sample areas and automated identification of microparticles. This advancement allowed for efficient detection of microscopic particles, contributing to valuable data collection from a substantial patient population.
Regarding the implications of titanium microparticles, the study found that tissue samples exhibiting higher concentrations of these particles showed altered gene expression, particularly in genes associated with inflammation and wound healing. The investigation identified 14 genes that could be impacted, although it remains uncertain whether the presence of titanium microparticles directly influences the local immune response or if variations in gene expression are indicative of individual differences in inflammatory conditions.
The researchers suspect that titanium particles may be released during the surgical installation of implants, specifically when the screw-shaped implant is inserted into the jawbone. Given this context, the study acknowledges the importance of examining differences in microparticle densities among various implant systems, as the surface structure of implants could affect the deposition of microparticles. This area is now highlighted as a critical focus for ongoing research.
