In 2023, the highest incidences were reported in Saarland (36.4%) and Hamburg (36.2%), while states like Saxony (25.6%) and Brandenburg (29.3%) exhibited significantly lower rates. These differences suggest that cultural and social factors play a role in the decision-making process surrounding childbirth.

The DGGG and the German Midwives Association note that smaller maternity units often have higher cesarean rates, attributed to inadequate staffing and insufficient training for handling emergencies.
