Starting July 1, pharmacies across Germany will be authorized to provide assisted telemedicine (aTM) services, marking a significant step in the digital transformation of healthcare. This new initiative enables pharmacies to support patients in accessing video consultations and structured assessments directly within their premises. The implementation follows an agreement between the German Pharmacists Association (DAV) and the National Association of Statutory Health Insurance Funds (GKV-Spitzenverband), aligning with Section 129, Paragraph 5h of the Social Code Book V.

The groundwork for assisted telemedicine was established in 2024 with the passage of the Digitalization Act by the federal government. The act was designed to improve the accessibility and quality of healthcare through technology-driven solutions. One of its core provisions allows pharmacies to facilitate video consultations between patients and healthcare professionals, especially for those who may lack the necessary technical resources or experience to participate independently.

Scope of Assisted Telemedicine in Pharmacies

Under the new system, pharmacies are able to offer three main services, each of which is eligible for a flat-rate reimbursement:

• Conducting a structured initial assessment to determine the suitability of a patient for a video consultation (Special PZN: 19816313).
• Facilitating a video consultation within the pharmacy (Special PZN: 19816336).
• Providing both the initial assessment and the video consultation in a single session (Special PZN: 19816342).

This framework aims to ensure that individuals with limited digital literacy or access to technology are not excluded from remote healthcare services. Pharmacies are expected to use simple yet secure technical setups to maintain patient privacy and data protection.

Payment and Billing Procedures

The remuneration structure for these services has been defined by an arbitration panel and subsequently reviewed and approved by the Federal Ministry of Health (BMG). The ministry conducted its review within the legally mandated timeframe and did not raise any objections, although it emphasized the need for ongoing improvement and adaptation of the agreement as telemedicine evolves.

Pharmacies will receive a VAT-exempt flat fee for each service rendered, with the amount determined by the date of service provision:

• From July 1, 2026, to June 30, 2027: 30 euros per service
• From July 1, 2027, to June 30, 2028: 25.50 euros per service
• From July 1, 2028, to June 30, 2029: 23 euros per service
• From July 1, 2029, onwards: 21.50 euros per service

Initially, pharmacies will document these transactions using a printed special voucher. However, the system is set to transition to an electronic billing format in the following year, streamlining the administrative process and further integrating telemedicine into the digital health infrastructure.

Regulatory Oversight and Future Development

The implementation of assisted telemedicine is subject to continuous evaluation and refinement, as mandated by the Federal Ministry of Health. The ministry has called for the ongoing development of standards to ensure the services remain effective and relevant as technology and patient needs evolve.

By integrating assisted telemedicine into everyday pharmacy operations, the German healthcare system aims to enhance access to care, particularly for populations that may be underserved by traditional digital health channels. The approach also supports healthcare professionals by creating a structured environment in which remote consultations can be conducted efficiently and securely.
