Traditionally, the effectiveness of metal screws, which play a critical role in orthopedic surgery, has been challenged by their potential to fail under everyday physical stress. Complications such as loosening or breakage of screws have posed significant risks. The newly established model, however, allows for precise predictions of failure risk by analyzing the bone's structural characteristics.
Utilizing high-resolution micro-CT scans, the research team conducted an analysis of 100 bone samples under ten different loading conditions. This comprehensive study, led by the Division of Biomechanics at Karl Landsteiner University of Health Sciences, identified two critical factors that account for up to 90% of the variations in screw stability. The findings enable a more reliable approach to screw implantation, ultimately preventing complications associated with improper placement.
According to the study, successful prediction of screw stability hinges on understanding the bone structure surrounding the implant site. The research team utilized a micro-computed tomography (CT) scanner to examine the bone morphology of the samples before and after screw implantation. Following the surgical procedure, the screws were subjected to rigorous load tests that simulated common activities such as walking and lifting.
The results indicated a direct correlation between the density and volume of bone and the support provided to the screws. Specifically, bones characterized by higher density and volume exhibited significantly enhanced stability for the screws, irrespective of the type of mechanical load applied.
During the experiment, the researchers tested 100 pig bone samples, which closely resemble human bone, under various loading scenarios. They measured six distinct bone parameters, identifying bone volume (BV) and the bone volume fraction (BV/TV) as significant predictors of the force required for screw failure.
From the data gathered, the research team developed two statistical models capable of predicting screw failure. The first model relies solely on bone volume, while the second combines multiple bone parameters for a more comprehensive analysis. Both models demonstrated remarkable accuracy, explaining 70-90% of the variance in screw failure forces across all test scenarios.
This predictive capability holds particular promise for improving surgical practices. By leveraging this new understanding, surgeons can more accurately identify the most stable positions for screws within the bone, thereby minimizing the risk of postoperative complications such as screw loosening or fractures.
The implications of this research are particularly significant for personalized orthopedic care, especially for older patients or those suffering from conditions like osteoporosis. The ability to predict screw stability with greater precision may lead to safer, more effective surgical interventions.
