A significant advancement in lymphoma treatment has emerged with the approval of Odronextamab (Ordspono®), a bispecific antibody designed to target follicular lymphoma (FL) and diffuse large B-cell lymphoma (DLBCL). These two forms represent the most common types of B-cell non-Hodgkin lymphoma, affecting thousands of patients annually in Europe.
Follicular lymphoma, although a slower-growing subtype, is considered incurable, leading to relapses in most patients after initial treatment. On the other hand, diffuse large B-cell lymphoma is known for its aggressive nature, with a progression probability reaching up to 50% in high-risk patients following first-line therapy. Each year, approximately 15,000 new cases of FL and 31,000 cases of DLBCL are diagnosed in Europe, underscoring the urgent need for effective treatment options.
Odronextamab is specifically approved for adult patients who have experienced relapsed or refractory FL or DLBCL after undergoing at least two prior treatments. This innovative therapy operates by binding to the CD3 antigen on T-cells and the CD20 surface antigen on B-cells, effectively bringing these two cell types into proximity. This spatial arrangement activates T-cells, prompting them to release proteins that destroy cancer cells.
The administration of Odronextamab is conducted via intravenous infusion, commencing with a gradual dose escalation over four cycles, each lasting 21 days. In the first cycle, patients receive infusions on days 1, 2, 8, 9, 15, and 16. For cycles two to four, infusions occur on days 1, 8, and 15. Following these cycles, a maintenance phase begins, where the antibody is administered biweekly. Should a patient show complete response for nine months, the interval between doses may be extended to every four weeks. Treatment continues until disease progression is observed or if unacceptable toxicity arises.
